New EHR integration settings

There are three new settings in Settings > CodeWriter Settings > Integrations > Redox Configuration that determine which type of PDF reports are uploaded to EHR: Code Record, Full Disclosure, and Vital Trend Report. If none of the options are selected, then the "Notes destination key" will be disabled.
